Health and Wellbeing Coach

Hours: Full-time, Permanent (Flexitime – Monday to Friday)

Location: Healthbox CIC – Ellesmere Port (One Location)

Salary: Up to £25,500 per annum (Pro Rata)

We are a not-for-profit Community Interest Company delivering a wide range of health and wellbeing services across Cheshire. Established for 11 years, the team comprises: Counsellors, Social Prescribing Link Workers, Nutritionists, Exercise Professionals, Oral Health Practitioners, Health Professionals and community Development Workers.

The Role

We are looking to fulfil the role of Health and Wellbeing Coach (HWBC). This role is an exciting opportunity for someone who is passionate about health and wellbeing to work alongside our existing team and One Ellesmere Port Primary Care Network (PCN). The PCN is a collaboration of the 6 GP Practices within Ellesmere Port who collectively care for over 70,000 patients.

 

The successful applicant will work with people, usually over a number of sessions, supporting them to develop their knowledge, skills and confidence, helping them become active participants in their own health and wellbeing. Providing guidance and support for people to reflect on and change what they do to help them reach their own health and wellbeing goals.

 

Often working with people with long-term conditions such as type 2 diabetes or chronic obstructive pulmonary disease (COPD). At other times working with those who are at risk of developing a long-term condition, providing support for issues such as weight management, managing chronic pain, or living with depression or anxiety.

 

The ability to coach, communicate and support behavioural change, and the ability to work with people at their own starting point, will encourage and support them to become more engaged in managing their own health.

In order to be effective in this approach, you will need to

  • Listen actively and show you understand what matters most to the person

  • Provide non-judgmental support

  • Reflect to people what you understand they have said, checking understanding

  • Put what matters most to the person at the heart of every conversation

  • Be warm and friendly

  • Treat each person with dignity and respect

  • Where a person is not happy with their support, enable them to make a complaint
     

Education

Any of the following qualification would be desirable:

  • Level 3 Exercise Referral

  • Registered Nutritionist

  • Essential – Nutrition related degree or equivalent

  • Bachelor's (required)

  • Certificate of Higher Education (required)
     

Experience

  • Desirable - Experience working in the health and wellbeing industry

  • Using motivational interviewing techniques

Licences/Certification

  • Essential – Full driving licence with access own transport

Social Prescribing Link Worker

Hours: Full-time

Location: Healthbox CIC – Cheshire South

Salary: £25,294 per annum (Pro Rata)

We are looking for an energetic and enthusiastic individual to join our Chester South Social Prescribing team in a full time role.


As a Social Prescribing Link Worker you would be helping to support and empower patients to take control of their health and wellbeing by giving patients time to focus on “what matters to me”. Link Workers use a holistic approach in supporting and connecting patients to groups and activities within their local community to improve and manage their wellbeing, tackle social isolation and meet their non-medical needs. Link Workers also support existing groups to be accessible and sustainable and help build new community groups, whilst working collaboratively with local organisations.


The Social Prescribing Link Worker will work as a key part of the Chester South Primary Care Network multi-disciplinary teams, our established team and wider community groups and organisations. 
We are ideally looking for someone with a background in health promotion, social prescribing or physical activity and/or group facilitation. The role requires the ability to travel across the Chester South footprint, and working within the GP practices part of the Primary Care Network.


Key role responsibilities to include
●    Manage a caseload of clients
●    Provide telephone, face-to-face and group-based support
●    Facilitate and support activities in the community
●    Keep up-to-date and accurate records, following Healthbox CIC data recording processes
●    Work as part of the Social Prescribing Team, Primary Care Network Team and Healthbox CIC
●    Have a good understanding of safeguarding and confidentiality (full training will be provided)
